(WESSINGTON SPRINGS, SD) – Horizon Health Care, Inc. (Horizon) is pleased to introduce Tia Chase, PA-C, as the newest medical provider at Jerauld County Community Health Center. Tia joins providers Dr. Lance Lim, MD, Dr. Michael Peña, MD, Dr. Thomas Dean, MD and Meghan Torgerson, CNP to provide primary care medical services at Jerauld County Community Health Center.
Chase attended the University of Sioux Falls, graduating in 2015 with a Bachelor’s Degree in Biology and Chemistry, and is a recent graduate of the University of South Dakota with a Master’s of Science degree in Physician Assistant Studies. Tia, who is currently available for appointments at Jerauld County Community Health Center, is originally from Huron, SD.
