Cassandra Aesoph, CNP, Joins Aberdeen Community Health Center

(ABERDEEN, SD) – Horizon Health Care, Inc. (Horizon) is pleased to welcome Cassandra Aesoph, CNP, to Aberdeen Community Health Center. Aesoph joins providers Danny Wolfgram, MD, and Catherine Friesen, CNP, to provide primary care medical services to the Aberdeen community.

Aesoph attended Presentation College in Aberdeen, graduating in 2012 with her Bachelor’s Degree in Nursing. After graduation, she worked as an RN before receiving her Master’s Degree in Nursing from Purdue Global University in 2019.

“I aim to build successful relationships with my patients so that they can be their best selves,” stated Cassandra Aesoph, CNP. “My ultimate goal is to make a positive difference in people’s lives.” Aesoph is currently available for appointments at Aberdeen Community Health Center

Aesoph, who is originally from Milbank, South Dakota, is married to husband, Mike. They recently welcomed a baby boy to the family. In her spare time, she enjoys spending time with her family.

Danny Wolfgram, MD, Joins Aberdeen Community Health Center

(ABERDEEN, SD) – Horizon Health Care, Inc. (Horizon) is pleased to welcome Danny Wolfgram, MD, to Aberdeen Community Health Center. Dr. Wolfgram joins providers Catherine Friesen, CNP, and Cassandra Aesoph, CNP, to provide primary care medical services to the Aberdeen community.

Dr. Wolfgram attended St. Olaf College and earned his bachelor’s degree in 1983 before earning his master’s degree at Iowa State University in 1985. He then attended medical school at the University of Minnesota – Duluth and the University of Minnesota – School of Medicine. Dr. Wolfgram earned his degree as a Physician in 1990. He specializes in family practice and sports medicine.

“As your physician, I am your partner in facilitating your medical care. As the patient, you are in control of accepting the care and making decisions to affect your medical future,” stated Danny Wolfgram, MD.

Dr. Wolfgram, a White Bear Lake, Minnesota native, has two sons, Alec and Jonathan, and one daughter, Elizabeth. His hobbies include curling, sailing, Taekwondo and spending time with his family. Dr. Wolfgram is now available for appointments at Aberdeen Community Health Center.

Horizon Health Care, Inc. Secures New Location in Aberdeen

After their negotiations fell through just weeks before their opening date, Horizon Health Care, Inc. (Horizon) is back on track with a new location.  With their new clinic located in the basement of the Johnson Professional Building, Horizon is excited to begin seeing patients on January 13th.

“We have definitely experienced our fair share of challenges, but we stayed positive and we are more than excited to have secured a location,” said Jeff Mengenhausen, Horizon’s Chief Operating Officer. “Thanks to RCE Properties, L.L.C. we are now on the fast track to finalizing our facility and begin seeing our patients.”

Horizon, a community-based network that provides affordable, high-quality primary healthcare throughout 18 medical and dental clinics in South Dakota, received federal funding September 13, 2013 to open a community health center in Aberdeen. The provided funding will help Horizon with their initial start-up costs, securing a permanent location, and continue to help with general operating support throughout the upcoming years.

With their new location, Horizon will provide the Aberdeen community with a wide range of quality health services such as family medicine, pediatrics, emergency treatment, x-ray, lab services and mental/behavioral health.

Horizon’s newest community health center will provide patients with personalized, quality, primary health care with the extra benefit of enhanced resources like the Sliding Fee Program and acceptance of most health insurance, including Blue Cross Blue Shield, Avera and Sanford.

Aberdeen Community Health Center is located at 422 5th Avenue SE, Aberdeen SD and will begin accepting appointments on January 13. Clinic hours will be 8 a.m. to 5 p.m. Monday through Friday.  If interested in scheduling an appointment, please call 605-725-3900.

Horizon Health Care, Inc. To Open Community Health Center in Aberdeen

Horizon’s newest community health center will provide patients with personalized, quality, primary health care with the extra benefit of enhanced resources like the Sliding Fee Program, participation with Avera and Sanford networks and acceptance of most health insurance. The funding will help Horizon cover start-up costs and allow Horizon to offset the costs of providing reduced patient fees through the Sliding Fee Program.

“Our main focus is to offer quality health care that is accessible to the community as a whole,” said John Mengenhausen, CEO of Horizon Health Care, Inc. “And with Horizon’s Sliding Fee Program, partnerships with providers in neighboring areas, as well as expanded medical services, we’re excited about being able to serve even more people in the Aberdeen and surrounding communities.”

The new community health center will provide the Aberdeen community with a wide range of health services such as family medicine, pediatrics, emergency treatment, x-ray, lab services and mental/behavioral health. The Aberdeen office will begin as a paperless office with the implementation of Horizon’s Practice Management/Electronic Medical Record and telemedicine capability.

Horizon hopes to address the growing need for dental access in South Dakota by partnering with local dentists to ensure patients have access to dental care.  When Horizon completes a partnership, low-income and Medicaid patients will be able to receive affordable primary and preventative care as needed.

Scheduled to begin serving patients 120 days from September 13, 2013, Horizon is actively looking for the location of the new health center and new staff recruitment is underway. Horizon’s existing providers in the surrounding areas will be available for training and to provide specialty services.
