(DE SMET, SD) – Horizon Health is pleased to welcome Shelby Schoenfelder, CNP, to Horizon Health De Smet. She will join Kristin Pratt, CNP, Kelli Smith, CNP, and Dr. Lance Lim, MD, to provide primary care services to the De Smet community.
Schoenfelder is a current resident of Iroquois, SD but grew up in Gettysburg. She attended South Dakota State University for her undergraduate degree and did her graduate work at Clarkson College in Omaha, Nebraska,
For two and a half years, Schoenfelder worked as a nurse practitioner at Sanford Health Brookings Acute Care Clinic. She was a nurse practitioner with Avera Medical Group Gettysburg for one and a half years.
“I am not originally from DeSmet but am looking forward to working at Horizon and getting to know and care for the community in which I now reside,” Schoenfelder said.
When not treating patients, Schoenfelder enjoys spending time with her husband Eric, son Bryson, and their two dogs Oakley and Mac. She also loves gardening, reading and spending time outdoors.
