Rachel Ilesanmi, FNP-BC, DNP
Horizon Health Care is pleased to welcome Rachel Ilesanmi, FNP-BC, DNP, as a new medical provider at Faith Community Health Center. Dr. Ilesanmi joins provider Valarie Brown, PA-C, to provide primary care medical services to the Faith community.
Originally from Nigeria, Dr. Ilesanmi received a master’s degree in nursing from St. Joseph’s College of Maine in 2017. In 2021, she graduated with a Doctorate of Nursing Practice from Walden University in Minneapolis. Throughout her course of education, she’s gained eight years of experience as a registered nurse and four years as a family nurse practitioner. She currently resides in Faith and is enjoying getting to know the community.
“I believe in holistic care – not just curing diseases but having a focus on attaining utmost wellness in individuals,” said Rachel Ilesanmi, FNP-BC, DNP. “I serve because I care, and I’m looking forward to serving the Faith community.”
Dr. Ilesanmi is married and has three children, ages 16, 13, and six. In her free time, she enjoys participating in outdoor activities, reading, and cooking. She is currently available for appointments at Faith Community Health Center during regular hours of operation on Monday through Thursday, 7:00 a.m. to 5:30 p.m., and on Friday, 8 a.m. to 5 p.m.
