Many Americans – young or old, male or female, rich or poor – suffer from oral disease. Left untreated, oral disease causes pain, lowers productivity and increases our risks for other diseases. Horizon’s dental facilities allow everyone easy and affordable dental care.

Providing our rural and medically underserved communities with access to preventive dental care saves money by reducing the incidence of untreated decay and dental ailments that cause people to seek treatment in emergency rooms. Horizon has eight dental locations with nine dentists in the following communities:

Mental/Behavioral Health

Horizon offers mental/behavioral health services for individuals, groups and families at all of our community health centers through telemedicine and on site contracted services. The goal of Horizon’s mental health services is to help people cope with current stressors in order to minimize the impact on their daily lives. Horizon strives to provide each of our communities a “Patient Centered Medical Home” and the inclusion of mental health care enhances our ability to improve care and reduce costs.



Telemedicine brings specialty health care directly to our patient’s communities. It decreases expenses and family stress by reducing travel time and costs. It also increases our patient’s access to a wide variety of health care services and specialties – all without having to leave the comfort and convenience of their own community.

Horizon is proud of its role in the application of its video conferencing technology for telemedicine and distance learning.  This technology allows live, interactive video connections among Horizon’s clinics and also with specialty healthcare providers in larger health systems in the state.  Over the past 10 years, Horizon has received four grants totaling over $2 million in funding for its telemedicine program.  Clinical services offered through telemedicine range from primary care to specialty services that include oncology, dermatology, pulmonology, and mental/behavioral health.
