(HOWARD, SD) – Horizon Health has added the BabyReady Medicaid Whole Pregnancy Care Program as part of maternal health services at its Mission, White River and Eagle Butte locations. The no-cost program covers all aspects of pregnancy including labor and delivery.
The BabyReady program aims to improve maternal health outcomes by enhancing care coordination, providing needed services aligned with best practices for standards of care and reducing barriers to care for patients and their families.
“The goal is for the patients to have sufficient prenatal care to have healthier moms and babies,” said Theresa Weidner-Eichstadt, Patient Care Coordinator for the maternal health program. “After baby is born, we want to continue to serve mom and baby in that postpartum period. We will collaborate with the services through the region and state to help get the best care possible for you and baby.”
The program covers health care costs for the mother and baby through the first 12 months of the infant’s life. BabyReady provides a mom-centered care team that is available to answer questions and give new parents a sense of safety and security about everything to come with the birth and first months of their baby’s life. The BabyReady program can connect parents with resources including food assistance, mental health services, transportation options, breastfeeding resources and dental care.
