(HOWARD, SD) – The Horizon Health Foundation would like to announce the winners of its Drive It Home ’26 Raffle.
The campaign was launched in January 2026 and succeeded in raising over $25,000 to positively impact the lives of patients throughout Horizon Health’s 28,000 square-mile footprint.
Brad Melstad was the first-prize winner in the Drive It Home ’26 Raffle. He won a 2026 Chevy Trax LT from Prostrollo Motors in Madison, SD. ile footprint.
The raffle campaign supported care kits for Horizon Health patients managing diabetes, cardiovascular and dental diseases. In 2025, 242 patients were enrolled in Horizon’s Chronic Care Management Program. The care kits will give additional assistance to these patients and any Horizon Health patient managing diabetes, cardiac or dental issues. The care kits will be free to patients and include equipment to manage their health they would otherwise have to purchase.
“It was an honor to meet so many incredible individuals across the Horizon Health footprint,” said Brady L. Kerkman, Director of Development for the Horizon Health Foundation. “Your support in this cause will have a lasting impact on those with chronic diseases. I am greatly humbled by how the communities came together to ensure access to high quality health care.”
Winners were drawn at the Horizon Health Foundation Office in the organization’s headquarters at Howard, South Dakota.
Lizbeth Gellerman of Yankton won the second prize, a half a hog and deep freeze.
