Our Mission: Health Care for Rural Communities … For You.
How to Give
Healthier Horizons »
Providing direct support to patients whether it is medical, dental or behavioral healthcare.
At least 20,000 patients every year will qualify for this support, helping them access the care they need, navigate tough financial situations, and find support for their overall well-being.
« Rooted in Rural
Helping to recruit health care professionals across the state.
South Dakota is expected to have one of the highest healthcare worker shortages of all 50 states by 2030. Without healthcare workers, clinic doors would shut within many of our communities. If there was no healthcare for families, it would have a devastating effect.
- Communities with no emergency care
- 60+ miles of traveling to receive care
- Families with small incomes unable to afford care
Horizon Impact Fund »
In rural communities, needs do not always arrive neatly labeled or fully funded. The Horizon Impact Fund exists to respond where the need is greatest, when it matters most. Your gift helps strengthen clinic operations across our communities, ensuring providers have the tools, equipment, and resources they need to deliver quality care in a community near you.
« Planned Giving
In setting aside a portion of your estate to benefit our community health centers, you leave a legacy of lasting care for your neighbors and friends. This gift lifts the burden of financial worry, untreated medical, dental and behavioral health conditions, and unsurety about quality of life. Thank you for considering planned giving and creating impactful care lasting more than a lifetime.
Giving Hearts Club »
Giving Hearts Club members are our most loyal donors. Their monthly commitment guarantees healthcare is a reality for EVERYONE no matter their income or geographic location.
By giving to this club, you provide patients with transportation assistance, food security, financial support, and supplies and equipment for their care.
These monthly gifts create healthcare sustainability through steady funding, allows you to give what fits in your budget, and makes it easy to make a lasting impact on patient lives.
« Smiles for Miles
Each October nearly 200 children ages 0-5 years old receive free dental care thanks to the Smiles for Miles program. Seeing a dentist in the first year of life is pivotal to creating beautiful smiles. For many parents, dental insurance doesn’t cover their youngest children. Smiles for Miles fills that gap and creates opportunities for parents who can’t afford dental care at all. Smiles for Miles has made it possible for more than 1,000 children to get a dental checkup, cleaning and fluoride treatment before entering kindergarten.
