Horizon Health Care is pleased to welcome Cynthia Boltz, CNP, as a new medical provider at Martin Community Health Center. Boltz joins providers Teresa Marts, MD; Mark Cali, PA-C; MaryAnn Fanning, CNP; and Jocelyn Bergh, PA-C to provide primary care medical services to the Martin community.
Originally from Wounded Knee, SD, Boltz received a bachelor’s degree in nursing from the University of Nebraska. In 2019, she graduated with a Family Nurse Practitioner degree from Bradley University. Throughout her career, she’s gained 19 years of experience in nursing and family medicine.
“Giving patient-centered care by meeting the psychological, physical, medical and culturally appropriate care is essential,” said Cynthia Boltz, CNP. “Striving to always practice kindness, respect, dignity and courtesy. Being committed to providing the best possible medical care.”
Boltz has three biological children and two foster daughters. In her free time, she enjoys hiking, swimming, cooking, and family time. She is currently available for appointments at Martin Community Health Center during regular hours of operation on Monday through Thursday, 8:00 a.m. to 6:30 p.m., and Friday, 8 a.m. to 5 p.m.
