Horizon behavioral health services provide support for individuals, groups, and families at all of our community health centers through telemedicine and onsite services. However, you must be a registered patient of Horizon Health Care to take advantage of our behavioral health services.
One goal of Horizon’s mental health services is to help you cope with current stressors in order to minimize the impact on your daily lives. Mental health care professionals strive to provide each of our communities a safe and welcoming home. Our behavioral health care team enhances our ability to improve overall care and reduce costs for patients.
During your visit, your primary care provider will ask about your physical and behavioral health including your mood, energy, sleep, and how you deal with stress. Together, as a team, we will best determine how to help you reach your goals and develop a plan for health and success.
Why do we ask this? Because we are committed to “whole person care”, or as it is often called “integrated behavioral health or collaborative care.” This type of care recognizes that both medical and behavioral health factors are important in your overall health.
