(Plankinton, SD) – Horizon Health Center is pleased to welcome Alyssa Hofer, CNP, as a provider at Aurora County Community Health Center.

For the last year Alyssa has worked in Horizon’s Plankinton, Wessington Springs, Woonsocket and Fort Thompson locations as a part-time provider. She transitioned to Aurora County CHC on August 1. She joins primary care provider McKenzie Bormann, PA-C, at Aurora County Community Health Center.
Hofer is originally from Mitchell, South Dakota, and received her bachelor’s degree as a registered nurse from Dakota Wesleyan University in Mitchell. In 2022, she graduated from South Dakota State University as a Certified Nurse Practitioner.
“I have enjoyed getting to know the community of Plankinton the last 3 years while completing my nurse practitioner clinicals at Horizon, working as a nurse and then transitioning to a part-time provider last fall. I am excited to continue to grow my relationship with my patients and offer high-quality, comprehensive care to all ages,” Hofer said.
In her free time, Hofer enjoys reading and crafting. She and her husband, Drew, have one son.
