Jennifer Franke, CNP, Joins Martin Community Health Center

(MARTIN, SD) – Horizon Health Care, Inc. (Horizon) is pleased to introduce Jennifer Franke, CNP, as the newest medical provider at Martin Community Health Center. Jennifer joins providers Francoise Bansenga, CNP-FNP, BC, Ashley Rook, PA-C and Dr. Tony Berg to provide primary care medical services at the Martin Community Health Center.

A graduate of Walden University, Franke holds a Master’s of Science in Nursing degree as a Family Nurse Practitioner. Prior to joining Horizon, Franke has been previously employed with Rapid City Regional Hospital as an ER Nurse and Cardiovascular Nurse.

Franke, who is currently available for appointments at Martin Community Health Center, is originally from Rapid City, SD. In her spare time, she enjoys camping, hiking, paddle boarding and other lake activities with her daughter, Genesis.

Autumn Tucker, CNP, Joins Mission Community Health Center

(MISSION, SD) – Horizon Health Care, Inc. (Horizon) is pleased to introduce Autumn Tucker, CNP, as the newest medical provider at Mission Community Health Center. Autumn joins providers Betty Kalblinger, PA, Larry Dubray, PA, Rachele Vietor, PA-C, Spencer Mason, PA-C, Dr. Tony Berg and Dr. Mark Butterbrodt to provide primary care medical services at the Mission Community Health Center.

A graduate of Walden University, Tucker holds a Master’s of Science in Nursing degree as a Family Nurse Practitioner. Prior to joining Horizon, Tucker has been previously employed with Winner Regional Health Care Center and also through R Vision Medical Staffing as a traveling RN.

Tucker will be available for appointments at Mission Community Health Center beginning March 28th. “I enjoy helping others,” said Tucker. “I treat all patients as I would want my family treated.”

Originally from Winner, SD, Tucker received her undergraduate degree from the University of South Dakota. In her spare time, she enjoys traveling, camping and fishing with her husband, Mark, and two sons, Gavin and Gage.

Kristin VanBuskirk, CNP, Joins James Valley Community Health Center

(HURON, SD) – Horizon Health Care, Inc. (Horizon) is pleased to introduce Kristin VanBuskirk, CNP, as the newest medical provider at James Valley Community Health Center. Kristin joins providers Len Wonnenberg, PA-C, Paul Brown, PA-C, Amanda Adams, PA-C, and Dr. Ruth Lim to provide primary care medical services at the James Valley Community Health Center.

A graduate of Mount Marty College in Yankton, SD, VanBuskirk holds a Master’s Degree in Pediatrics and a Post Master’s Degree in Family Practice. Prior to joining Horizon, VanBuskirk has been previously employed with the South Dakota Department of Health. Most recently, she was employed as a Nursing Instructor at Presentation College in Aberdeen. “I love my profession! There is always a new opportunity to be the best part of someone’s worst day,” said VanBuskirk. “Helping others has always been a passion of mine.”

A Hitchcock native, VanBuskirk enjoys playing volleyball, traveling and camping with her husband, Patrick, three sons, Caden, Sawyer and Carson, and daughter Karter.

VanBuskirk will be available for appointments at James Valley Community Health Center beginning the week of March 20th. “I enjoy working in small rural communities because you become family with your co-workers and personable with patients,” said VanBuskirk.

Tony Berg, MD, Joins Southwest Region Clinics

(MISSION, SD) – Horizon Health Care, Inc. (Horizon) is pleased to introduce new medical provider, Tony Berg, MD, to the southwest region communities of Mission, Martin and White River.

Berg is a Winner, SD native, with thirty-five years of experience in the community health center setting in South Dakota. He attended South Dakota State University in Brookings, SD, graduating with a Bachelor’s Degree in Zoology and went on to receive a Doctorate of Medicine degree from the University of South Dakota School of Medicine and the University of Alabama.

Tony has been a contracted physician with Horizon for many years and will be a great addition to our health care team in this region,” said Jessica Weber, Horizon Regional Office Manager. “We look forward to having him on staff at all three clinics to provide the best comprehensive care for our patients.”

Berg joins a team of eleven current providers in the region and is available for appointments several days a month at Mission Community Health Center, Martin Community Health Center and Mellette County Community Health Center.

Horizon Health Care, Inc. (Horizon) is pleased to introduce new medical provider, Ashley Rook, PA-C, to the Martin community.

Ashley Rook is a Rapid City, SD, native, with three years of experience in the community health center setting in South Dakota. She attended South Dakota School of Mines and Technology in Rapid City, SD, graduating with a Bachelor’s Degree in Chemistry and went on to receive her Master’s Degree in Physician Assistant Studies from the University of Pittsburgh.
“Ashley started with us in December and has been a great addition to our health care team at Martin Community Health Center,” said Jessica Weber, Horizon Regional Office Manager. “We are excited for her to meet our patients in the Martin community and know that she will strive to provide the best comprehensive care.”
Rook joins a team of four current providers (Tiffany Spears, NP-C, Anna Waldner, CNP, Francoise Bansenga, FNP-BC, and Dr. Tony Berg) and is available for appointments, Monday-Thursday.

Donna Linke, CNP, MSN, Joins Horizon Health Care, Inc.

Horizon Health Care, Inc. (Horizon) is pleased to introduce Donna Linke, CNP, MSN, as a Psychiatric Nurse Practitioner. Linke will be located at Horizon’s Wessington Springs and Fort Thompson locations, but will provide mental health services via telemedicine to all of Horizon’s locations.

A graduate of University of Missouri-Columbia, Linke holds a Master’s of Science in Nursing degree with a specialization in Psychiatry. She has had extensive experience in the counseling and psychiatric fields; most recently, she was employed with Fort Thompson Indian Health Center in Fort Thompson, SD.

” I am very blessed to have spent the past six years working with the Hunkpati Oyate on the Crow Creek Reservation,” said Donna Linke, CNP, MSN. “My career with Horizon will allow me to continue working closely with the communities on the Crow Creek Reservation, which has become my ‘home away from home.’ It will also enable me to share my expertise and expand psychiatric services across rural South Dakota in both Native and non-Native communities.”

Originally from Woonsocket, SD, Linke received her undergraduate degrees from Huron University. Linke is married and her family includes two stepchildren, four grandchildren, plus two dogs and three cats. In her spare time, she enjoys baking, cooking, canning, hunting, fishing, and camping.

Linke will be available for appointments Tuesday-Friday from 8:00-5:00 p.m. Appointments can be scheduled by calling the Jerauld County Community Health Center.

Francoise Bansenga, FNP-BC, Joins Martin Community Health Center

(MARTIN, SD) – Horizon Health Care, Inc. (Horizon) is pleased to introduce Francoise Bansenga, FNP-BC, as their newest provider at Martin Community Health Center, located in Martin, SD.

A recent graduate of Mount Marty College in Yankton, SD; Bansenga holds a Master’s of Science in Nursing Degree with a specialization in Family Practice and brings over ten years of experience in the medical field to her position at Martin Community Health Center. She has extensive experience in various hospital settings across South Dakota, Iowa, Texas, and Kansas, specializing in geriatrics, acute care, adult and pediatric health. Most recently, she was employed at Sanford Health in Sioux Falls, SD, while furthering her education.

“I have always been interested in addressing health disparities and thought nursing would be the best way to do that,” said Bansenga. “I believe everyone deserves access to quality health care. I am very excited to join Horizon’s team in Martin as they meet a huge need for care in the area.”

Originally from Sioux Falls, SD, Bansenga received her undergraduate degrees from University of South Dakota and Mount Marty College. Bansenga is married and the mother of five children; in her spare time, she enjoys biking, exercising, reading, and staying up-to-date with current global and domestic affairs.

Bansenga joins a team of four current providers (Dr. Mark Butterbrodt; Nancy Webb, FNP; Tiffany Spears, NP-C; and Anna Waldner, CNP) and will be available for appointments beginning this week, Monday-Friday, 9:00 a.m. – 6:00 p.m.
