Katlyn Rostyne, CNP, Joins Martin Community Health Center

(Martin, SD) – Horizon Health Care, Inc. (Horizon) is pleased to introduce Katlyn Rostyne, CNP, as the newest medical provider at Martin Community Health Center. Katlyn joins providers Dr. Tony Berg, MD, Margaret Lee, PA-C, Theresa Hammerquist, CNP, MaryAnn Fanning, CNP and Nancy Webb, CNP, to provide primary care medical services to the Martin community.

Rostyne attended Dakota Wesleyan University in Mitchell, SD, and earned a Bachelor’s Degree in Nursing. She later went on to attend Maryville University of St. Louis in St. Louis, MO, graduating in 2018 with a Master’s of Science in Nursing as a Nurse Practitioner.

“I believe that everyone has personal health goals that are achievable,” said Katlyn Rostyne, CNP. “I hope to provide patients with education and support to meet their health care expectations.”

Originally from Huron, SD, Katlyn enjoys camping, fishing, softball and spending time with her family. She is currently available for appointment at Martin Community Health Center.

MaryAnn Fanning, CNP, Joins Martin Community Health Center

(Martin, SD) – Horizon Health Care, Inc. (Horizon) is pleased to introduce MaryAnn Fanning, CNP, as the newest medical provider at Martin Community Health Center. MaryAnn joins providers Dr. Tony Berg, MD, Margaret Lee, PA-C, Theresa Hammerquist, CNP, and Nancy Webb, CNP, to provide primary care medical services to the Martin community.

Fanning attended the University of South Dakota in Vermillion, SD, and earned an Accredited Associate’s Degree in Nursing. She later went on to attend the University of Mary in Bismarck, ND, graduating in 2009 with a Master’s of Science in Nursing. In 2013, she earned a Post Master’s Degree as a Family Nurse Practitioner from Kaplan University.

“I am excited to be a part of the Horizon team, to learn and share my experiences,” said MaryAnn Fanning, CNP, who is currently available for appointments at Martin Community Health Center.

Originally from Martin, MaryAnn resides with her husband, Paul, who is a rancher in Bennett County. In her spare time she enjoys music, drama productions, traveling and spending time with her grandchildren.

Theresa Hammerquist, CNP, Joins Martin Community Health Center

(Martin, SD) – Horizon Health Care, Inc. (Horizon) is pleased to introduce Theresa Hammerquist, CNP, as the newest medical provider at Martin Community Health Center. Theresa joins providers Dr. Tony Berg, MD, Margaret Lee, PA-C, and Nancy Webb, CNP, to provide primary care medical services to the Martin community.

Hammerquist attended University of South Dakota in Vermillion, SD, and earned a Bachelor’s Degree in Psychology and Spanish. She later went on to attend South Dakota State University, graduating in 2008 with a Bachelor of Science in Nursing Degree. In 2017, she graduated from Georgetown University with a Master’s of Science in Nursing.

“I chose to earn my advanced degree in nursing to learn more and to provide care to those in rural settings. I believe in the team approach where patient and provider work as a team to guide prevention and care with the ultimate goal for healthier living,” said Theresa Hammerquist, CNP, who is currently available for appointments at Martin Community Health Center.

Hammerquist, who is originally from Rapid City, SD, enjoys cooking and having company at the dinner table. In her spare time she also enjoys hiking and kayaking with her family.

Margaret Lee, PA-C, Joins Martin Community Health Center

(MARTIN, SD) – Horizon Health Care, Inc. (Horizon) is pleased to introduce Margaret Lee, PA-C, as the newest medical provider at Martin Community Health Center. Margaret joins providers Francoise Bansenga, CNP-FNP, BC, Jennifer Franke, CNP, Courtney Carmichael, PA-C and Dr. Tony Berg, MD to provide primary care medical services at the Martin Community Health Center.

A graduate of the University of South Dakota, Margaret holds a Master of Science degree in Physician Assistant Studies. Lee, who is currently available for appointments at Martin Community Health Center, is originally from San Diego, CA.

“Our family was among the many Chinese families who piled outside the gate of the U.S. Embassy as American troops were withdrawing from Vietnam. The American people didn’t forget about my family, but saved us and brought us here to safety,” said Margaret Lee, PA-C. “I want to give back to the people who sacrificed their own lives to preserve mine. I realized we are all connected in wonderful ways. I lived with a farmer/rancher family during one of my clinical rotations in Avon last year, and learned that each farmer and rancher here in the U.S. is carrying the country on their backs. I want to be part of the healthcare team who takes care of these people that toil night and day. I’ve come to Martin to learn from the people, to grow with the community and hopefully to take good care of the families along the way.”
