Horizon Health Care, Inc. and allPOINTS Health Services Merger Final

(HOWARD, SD) – As of June 1, 2017, Horizon Health Care, Inc. (Horizon) and allPOINTS Health Services have successfully completed the previously announced merger that will expand Horizon’s network to include 31 medical and dental clinics across the state of South Dakota.

All located in southeastern South Dakota, Horizon will now operate: Alcester Community Health Center, Alcester Dental Clinic, Elk Point Community Health Center, and Yankton Community Health Center and Dental Services.

While the clinics will see a few visual changes outside with new logos and signage, patients can be assured that the providers and staff they have formed relationships with over the years will still be providing quality care. “We are pleased with how smoothly the preparations for this merger went and are eager to continue building relationships in these communities that allPOINTS Health Services has created throughout the years,” said John Mengenhausen, Horizon Health Care, Inc. CEO. “We have a great team in our five newest Horizon locations and we will remain committed to serving our rural patients who deserve affordable health care.”

Many of the clinics will be changing hours as of June 5th to offer a more consistent, open schedule. Details regarding these hours can be found on the website and in local media outlets.

This merger will bring new opportunities to Horizon’s southeastern sites in years to come, including access to Horizon’s telemedicine capabilities. These telehealth services will provide patients with expanded access to services and communications with mental health professionals and specialty physicians, without having to leave their local clinics and travel long distances.

An open house and ribbon cutting celebration will be held in each location in mid-August to commemorate the merge and introduce Horizon Health Care to each of the communities.

Horizon Health Care, Inc. and allPOINTS Health Services Merger on track for June 1, 2017

(HOWARD, SD) – Horizon Health Care, Inc. (Horizon) and allPOINTS Health Services (allPOINTS) are on track to merge their organizations on June 1, 2017; a date previously announced in January 2017. The respective community health centers will form a network of 31 medical, dental, and school-based clinics throughout the entire state of South Dakota that will operate as Horizon clinics.

The clinics will not be closed during the merger transition, but there are a few changes that can be expected at the time of the transition. The allPOINTS clinics will be changing names to: Alcester Community Health Center, Alcester Dental Clinic, Elk Point Community Health Center, and Yankton Community Health Center and Dental Services, to be consistent with the names of other Horizon clinics. However, the phone numbers and addresses for each of the facilities will remain the same.

Patients can expect to be greeted by their same primary care provider and support staff that they are accustomed to through the transition, but there will be some new faces added to the clinics. Dr. Nelson Batoon and Anna Waldner, CNP, both current Horizon providers, will be transferring to the Elk Point, Alcester, and Yankton clinics and will begin seeing patients on Monday, June 5th. Dr. Batoon will see patients at all three locations and Anna Waldner will be available in Alcester. A former familiar face with allPOINTS, Kristin Morrow, CNP, will be returning to the organization at the time of the merger and will be located in Elk Point. She previously served patients in Alcester while she was with allPOINTS. Other current allPOINTS providers, Traci Holmquist, CNP, Holly Haberman, CNP, Dr. Paul Cortez, and Jack Creelman, Hygienist will all continue to see patients in their respective locations.

Horizon has also hired a new Regional Office Manager, Janet Fulk, to oversee the Southeastern medical and dental clinics currently operated as allPOINTS. Janet had been overseeing the Physician Assistant’s program at USD since 2004, prior to joining Horizon. She has already started training with the organization and will begin managing the facilities at the time of the merger.

HOPE $20,000 Match Challenge Raises Nearly $13,000 for South Dakota Patients

(HOWARD, SD) – Donors across South Dakota joined with Creative Printing of Huron, SD to raise nearly $13,000 for Horizon Health Care patients unable to afford a colonoscopy.

In honor of Colorectal Cancer Awareness Month in March, Creative Printing owners Brad and Laura McGirr partnered with the Horizon Health Foundation to offer the HOPE $20,000 Match Challenge to double gifts to support South Dakota patients.

Sixty-three donors contributed to the challenge, which was designed to raise money for patients, as well as raise awareness about the importance of screenings and how they can save lives. All funds contributed to the challenge were matched by Creative Printing.

“The Foundation Board of Directors, along with our Horizon staff, is grateful for the impact made from this partnership with Creative Printing and the generosity of our donors,” said Tracy Pardy, Horizon Health Foundation Director. “This challenge has been a wonderful example of South Dakota individuals and businesses coming together to make a difference and support their neighbors and colleagues across the state.”

In 2016, over 500 Horizon Health Care patients completed an at-home colorectal cancer screening test. Of those, 119 tested positive and needed a follow-up screening. Funds raised from this match challenge will directly benefit patients and will be used to provide colonoscopies to individuals needing the follow-up screening, but unable to afford it.

Howard Farmers Co-op Invests in Colorectal Cancer Prevention for Rural South Dakotans

(HOWARD, SD) – With limited days remaining to donate to the HOPE $20,000 Match Challenge, Howard Farmers Co-op has demonstrated the “neighbors helping neighbors” mentality by joining the Horizon Health Foundation’s efforts in providing life-saving care to South Dakotans.

Howard Farmers Coop

Horizon Health Foundation is pleased to announce a gift from Howard Farmer’s Co-op, in support of the $20,000 Match Challenge, which will benefit your South Dakota friends and neighbors who cannot afford a colonoscopy.

Howard Farmers Co-op is led by General Manager Bruce Yanish and a seven-member board of directors, who made the decision to donate to the challenge to raise awareness, while making a statement of support to their employees, customers, and everyone in our agricultural communities that have been or may be affected by colorectal cancer.

“As one of the largest employers in town, we felt that it was important to support this cause that can affect so many in our rural communities,” said Norman Andenas, Howard Farmers Co-op Board of Director. “These are our customers and neighbors that we could be helping. Anyone can suddenly find themselves without health insurance or unable to pay for their health care through some twist of fate.”

During the month of March, Creative Printing of Huron has partnered with the Horizon Health Foundation to offer this $20,000 Match Challenge. This challenge coincides with Colorectal Cancer Awareness month and all gifts made through the challenge will be directed to patients in need of a colonoscopy. “Colon cancer and colonoscopies aren’t an everyday topic of discussion, but they’re something everyone has experienced and should be talking about,” said Yanish. “Either you’ve experienced it yourself or someone you know has…this challenge is an opportunity to give a gift to spark more conversation on the topic.”

In these final days of March, individuals and businesses, have the opportunity to join Howard Farmers Co-op in making a gift to the Horizon Health Foundation in support of colorectal cancer prevention and awareness – a gift that will be matched to double the number of South Dakotans that will be touched with a potentially life-saving colonoscopy.
To join in this special opportunity to have your support make twice the difference, please visit horizonhealthfoundation.com/colon-care or call 605.772.5907 before March 31st.

Horizon Health Foundation Announces HOPE $20,000 Match Challenge to Support South Dakota Patients

Horizon Health Foundation, a non-profit serving Horizon Health Care patients, announces a partnership with Creative Printing and Huron Regional Medical Center to support South Dakotans in need. Creative Printing will offer a $20,000 Match Challenge now through March 31, 2017 to double gifts to support South Dakota patients who cannot afford the life-saving colonoscopy.

In honor of National Colorectal Cancer Awareness Month, these partners are joining forces to ask fellow South Dakotans, businesses, and organizations to donate to colorectal cancer prevention and awareness. “Without Horizon Health Care, many of our rural and frontier communities wouldn’t have access to medical care. They reach thousands of people across South Dakota. We know where our money is going—directly into the hands of people who need it. We support what we believe in, and we believe in our local community. If we save one life, this challenge will be successful,” said Laura McGirr, co-owner of Creative Printing.

Tracy Pardy, Horizon Health Foundation’s Director, adds, “This $20,000 Match Challenge is truly an example of neighbors helping neighbors. This partnership between Horizon and Huron Regional Medical Center shows how community health centers can join together with other healthcare providers to do greater good.”

To donate, givers are asked to visit the Horizon Health Foundation website. Every amount is welcome, and every gift matters to South Dakotans in need. The Horizon Health Foundation ensures that all funds donated and matched by Creative Printing will be directed to patients in need of this service.
