Horizon Receives Grant to Improve Rates of Cancer Screening

Horizon Health Care, Inc. (Horizon) has been awarded a grant from the South Dakota Department of Health for $19,493.76 to increase the breast, cervical and colorectal cancer screening rates in the communities of Alcester, Elk Point and Yankton, SD.

Horizon will be working with the South Dakota Department of Health Comprehensive Cancer Control Program and All Women Count! Program to develop strategies to ensure all people of appropriate age are screened.
According to the South Dakota Cancer Registry, cancer was the second leading cause of death in South Dakota in 2014 with 4,498 cancer cases diagnosed and 1,679 deaths. Each day, 12 cases of cancer are diagnosed in South Dakota residents, and four South Dakotans die from the disease. In 2014, the most common cancer diagnosis in the state was female breast cancer.

Many cancers are survivable if found and treated in their early stages. Completing recommended cancer screening tests helps find breast, cervical and colorectal cancer before they have advanced.
Horizon will utilize grant funds to improve breast, cervical and colorectal cancer screening rates by increasing awareness for the need for screening, sending reminders to patients who need to complete screening and working in partnership with clinicians to review rates of screening and develop strategies to increase screening rates among targeted populations.

Randall Law, PA-C, Joins Aberdeen Community Health Center

(ABERDEEN, SD) – Horizon Health Care, Inc. (Horizon) is pleased to introduce new medical provider, Randall Law, PA-C, to the Aberdeen community. Randall joins provider Leah Bennett, PA-C to provide primary care medical services at Aberdeen Community Health Center.

Law attended Brigham Young University – Idaho, graduating in 2014 with a Bachelor’s Degree in Biology, and is a recent graduate of Idaho State University with a Master’s Degree in Physician Assistant Studies.

“I am excited to join the outstanding team already in place at Aberdeen Community Health Center in providing medical care to the people of this great region,” said Randall Law, PA-C. “As a recent graduate, I don’t know everything. However, I am dedicated to learning all that I can in order to best serve those who place their trust in me. I look forward to working closely with patients to enhance and improve their lives.”

Law, a St. Anthony, Idaho native, is currently available for appointments at Aberdeen Community Health Center. His hobbies include playing and watching sports, exploring the outdoors, playing guitar and spending time with his family.

Courtney Carmichael, PA-C, Joins Martin Community Health Center

“I am very excited to begin my career in Martin, SD,” said Courtney Carmichael, PA-C. “My philosophy is to take care of the patient and everything else will follow.”

A recent graduate of the University of South Dakota, Carmichael holds a Master of Science degree in Physician Assistant Studies. Carmichael, who is currently available for appointments at Martin Community Health Center, is originally from Rapid City, SD. In her spare time, she enjoys reading, volleyball and spending time in the Black Hills.

De Smet Dental Services Welcomes Outreach Students

De Smet Dental Students(DE SMET, SD) – Horizon Health Care, Inc. (Horizon) and De Smet Dental Services, in collaboration with the University of Minnesota, are pleased to introduce two dental students who are doing their clinical rotations in De Smet from September 11 – October 9.

The two students visiting De Smet are Jake Stricherz of South Shore, SD, and John Weber of Milbank, SD. Jake attended University of Minnesota for his undergraduate degree, and then joined the Air Force, prior to moving back to Minneapolis for dental school. He enjoys spending time with family and friends, and watching his younger brother play football. After dental school, he plans to work in Western MN for a small practice.

John completed his undergraduate degree at the University of South Dakota in Vermillion. He enjoys hunting, fishing, hiking, traveling, and anything that involves the outdoors. When he was younger, John participated in many wrestling and basketball tournaments in the De Smet community. After graduation, John plans to come back to South Dakota to join a dental practice. “I am very excited to return to my home state to serve and gain experience in De Smet,” said John.

“It is a pleasure to once again welcome the dental students from UMN to De Smet,” said Dr. Michelle Scholtz, Chief Dental Officer of Horizon. “Our partnership has been very beneficial to Horizon, as we have been fortunate to hire six dentists (including Dr. Brady Weispfennig and Dr. Lamar Koistinen of De Smet Dental Services) to work at our dental clinics after their rotations.  These dental rotations are a great opportunity to bring dental students back to our rural communities.”

Horizon Health Care, Inc. Recognized as Health Center Quality Leader

(HOWARD, SD) – In a recent announcement by the U.S. Department of Health and Human Services, Horizon Health Care, Inc. (Horizon) has been named a Health Center Quality Leader and is one of four health centers in South Dakota to receive funding dedicated to recognize their health center’s quality improvement efforts.

Locally, Horizon will receive nearly $90,000 in recognition of their continuous quality improvement efforts. The purpose of this award is to support health centers that displayed high levels of quality performance in 2016 to continue to strengthen quality improvement activities. Health centers receive these funds based upon high levels of performance in one or more of the following categories: Improving Quality of Care, Increasing Access to Care, Enhancing Delivery of High Value Health Care, Addressing Health Disparities, and Achieving Patient-Centered Medical Home (PCMH) Recognition.

“Horizon is very pleased to be recognized again for the high quality of care our patients receive,” said Lindsey Karlson, Horizon’s Director of Quality Improvement. “As community health centers, we provide care to patients from diverse backgrounds and rural communities, many of whom would not have access to healthcare otherwise. A graduate of the University of Minnesota – School of Dentistry, Dr. Haley is originally from Mitchell, SD. Prior to joining Horizon, she previously owned Dental Health Partners in Mitchell.

“My goal is to provide comprehensive, gentle care to my patients,” said Dr. Hilary Haley, DDS. “I want to help guide and educate them, so that we can become partners in their dental health.”

Prairie Winds Dental Clinic Welcomes Outreach Students

(HOWARD, SD) – Horizon Health Care, Inc. (Horizon) and Prairie Winds Dental Clinic, in collaboration with the University of Nebraska – Lincoln, are pleased to introduce two dental students who are doing their clinical rotations in Howard from July 31 – August 18, 2017.

The two students visiting Howard are Michael Lindsay of Madison, SD, and Erica Dickmeyer of Fremont, NE. Lindsay’s plans after dental school include a pediatric residency. His hobbies include snowboarding, traveling, food and photography. Dickmeyer, who plans to move to Colorado or Kansas City after dental school, enjoys running, biking, playing tennis and reading.

“This program has been a great opportunity to bring dental professionals back to South Dakota to work in our rural communities,” said Dr. Michelle Scholtz, Chief Dental Officer of Horizon. “The rotation is preferred for students that are residents of South Dakota or neighboring states and we have been fortunate to have six dentists that have come to work for Horizon after their rotations. We always enjoy the time getting to know these students and introducing them to our communities.”

A recent graduate of the University of Mary, Nehl holds a Doctorate of Nursing Practice degree. Her hobbies include spending time on her family’s cattle ranch, riding horse, hunting and playing bass and singing in her band, The Broken Spokes.

Jennalee Olsen, CNP, Joins Aurora County Community Health Center

(PLANKINTON, SD) – Horizon Health Care, Inc. (Horizon) is pleased to introduce Jennalee Olsen, CNP, as the newest medical provider at Aurora County Community Health Center. Jennalee joins current provider Melissa Davis, CNP to provide primary care medical services at Aurora County Community Health Center.

A graduate of Clarkson College, Olsen holds a Master’s of Science in Nursing degree as a Nurse Practitioner. Prior to joining Horizon, Olsen has been previously employed with Sanford Chamberlain Medical Center as a Registered Nurse.

“I believe in providing quality, holistic care to each patient as if they were part of my own family,” said Jennalee Olsen, CNP, who is currently available for appointments at Aurora County Community Health Center.

Olsen is a White Lake native, who has made her home in Plankinton. She completed her undergraduate studies at the University of South Dakota, graduating with a degree in Nursing. In her spare time, she enjoys camping, boating, crafting and spending time with her husband, Beau and daughter, Hanley.
