Horizon Health Foundation Announces HOPE $20,000 Match Challenge to Support South Dakota Patients

Horizon Health Foundation, a non-profit serving Horizon Health Care patients, announces a partnership with Creative Printing and Huron Regional Medical Center to support South Dakotans in need. Creative Printing will offer a $20,000 Match Challenge now through March 31, 2017 to double gifts to support South Dakota patients who cannot afford the life-saving colonoscopy.

In honor of National Colorectal Cancer Awareness Month, these partners are joining forces to ask fellow South Dakotans, businesses, and organizations to donate to colorectal cancer prevention and awareness. The Huron Regional Medical Center has partnered with Horizon by offering a colonoscopy at a reduced rate to eligible participants. Last year over 500 Horizon Health Care patients completed an at-home colorectal cancer screening test. Of those, 119 tested positive and need a follow-up screening.

“Without Horizon Health Care, many of our rural and frontier communities wouldn’t have access to medical care. They reach thousands of people across South Dakota. We know where our money is going—directly into the hands of people who need it. We support what we believe in, and we believe in our local community. If we save one life, this challenge will be successful,” said Laura McGirr, co-owner of Creative Printing.

This partnership between Horizon and Huron Regional Medical Center shows how community health centers can join together with other healthcare providers to do greater good.”

Horizon Health Care, Inc. and allPOINTS Health Services to Merge

(HOWARD, SD) – Horizon Health Care, Inc. (Horizon) and allPOINTS Health Services (allPOINTS) board members and leadership are pleased to announce the intended merger of their respective community health centers to form a network of 31 medical, dental, and school-based clinics throughout the entire state of South Dakota.

“After months of discussion leading up to this decision, we are pleased with this opportunity to join forces with allPOINTS and add five well-established medical and dental clinics to our network,” said John Mengenhausen, CEO of Horizon Health Care, Inc. “Both organizations have a rich history in striving to improve the quality of life for those we serve through delivery of affordable health care in many of our state’s most rural areas. Horizon will continue to provide allPOINTS Health Services’ patients with access to the same quality health care they currently receive.”

allPOINTS Health Services is a Community Health Center with five medical and dental clinic locations in the communities of Alcestor, Elk Point, and Yankton, SD. Horizon and allPOINTS have been fortunate to have a long-standing relationship throughout the years, especially over the past fifteen years. Since 2003, Horizon has provided direct support to allPOINTS in the area of information technology, and most recently, the organizations have shared a partnership in the Prairie Health IT Network that provides shared IT staffing and health information technology training to staff.

Once the merger is complete, the allPOINTS clinics will operate as a part of Horizon Health Care, Inc. The current allPOINTS employees and providers will continue to serve patients at each clinic site. AllPOINTS Board Chair, Jerry Miller, has served on the board for the past five years and believes the merger will strengthen the services current patients are accustomed to. “We have given this decision a lot of consideration and wanted to move in the direction that was best for the nearly 3,000 patients we serve each year,” said Miller. “As a board, we would like to express our sincere appreciation to our patients and staff for their support over the past years. We have had great success in recent partnerships with Horizon and I feel confident that they will continue to strive for excellence.”

As the organizations work to integrate their locations and services, Mengenhausen and Sally Rosin, Horizon Board Chair, hope to have the merger completed by June 2017. “Throughout this upcoming integration period, Horizon is dedicated to merging the two organizations with minimal disruptions to patient care. As a board, we strongly believe that leveraging the strengths in both organizations will allow Horizon to improve and align processes in the delivery of quality patient care. We have a very exciting road ahead of us,” said Rosin.

Howard Community Health Center to Collect Outer Wear for Children

(HOWARD, SD) – A Mitten Tree to collect warm outer wear for children in need in the Howard area is on display in the waiting room at Howard Community Health Center. This tree was a long running project by Avera Medical Group Howard staff and has been taken over by Horizon Health Care this year.

Throughout the years, this project has been well supported by the community and thanks to the donations; many children in the area were gifted mittens, gloves, hats, and scarves. Families, individuals, clubs, church groups, businesses, and others are invited to purchase outerwear suitable for boys or girls, aged kindergarten through high school; the items may be brought to the clinic during business hours and hung on the Christmas tree. The goal for staff is to “fill up the tree”. Shortly after Christmas, the items will be taken to the Howard School and they will work with the teachers and staff to provide outerwear to the kids who are going outside without appropriate clothing.

“We are excited to continue this project with Horizon,” said Misty Rudebusch, PA-C. “We invite our community to join us this holiday season to bring warmth to the children of our community.”

Dannica Callies, DNP, NP Named Horizon Health Care Provider of the Year

(LAKE PRESTON, SD) – Dannica Callies, DNP, NP was honored with Horizon Health Care, Inc. (Horizon)’s inaugural Provider of the Year Award last week at the organization’s annual All Provider Meeting.

The Provider of the Year Award recognizes one medical or dental provider each year who embodies the spirit of care Horizon strives to provide to its patients and communities. Selection is based on a number of items, including patient satisfaction, quality of care, service to the community, and effective leadership.

Dannica has worked for Horizon’s Lake Preston Community Health Center for three years. In that time, she has dedicated herself to her patients, their families, and the community of Lake Preston. She also provides services in the De Smet Community Health Center and through telemedicine to patients in Martin, SD.

Fort Thompson Community Health Center Celebrates One Year Anniversary

(FORT THOMPSON, SD) – Horizon Health Care, Inc. (Horizon)’s Fort Thompson Community Health Center is celebrating one year of service this month.

On September 1, 2015 the Fort Thompson Community Health Center opened their doors to provide quality health care to the Fort Thompson and Crow Creek Reservation area. Led by Sarah Newton, PA-C, the clinic offers a full range of health services, including: family medicine, pediatrics, lab services, immunizations, free sports physicals, and mental/behavioral health. They also offer additional resources, such as the Sliding Fee Program and 340B Prescription Drug Program, as well as accept most health insurances.

Since opening last year, Horizon has been steadily working to bring additional services to the Fort Thompson CHC. This fall, they will begin offering OB services through Sanford in Chamberlain, with OB appointments available at the clinic. They also recently added Donna Linke, CNP, MSN, to the team as a Psychiatric Nurse Practitioner to provide mental health services.

“We would like to thank our wonderful patients, community, colleagues, and business partners as we celebrate one year in business,” said Newton and her Fort Thompson care team. “We’ve built many great relationships already and we’re looking forward to strengthening them further. Thanks for letting us help serve the medical community in and around the Fort Thompson area.”

Construction in Progress for Horizon Health Care’s New Faith Dental Clinic

(FAITH, SD) – Horizon Health Care, Inc. (Horizon)’s new Faith Dental Clinic project is moving forward and on track to be completed in mid-October.

Renovations began in mid-August to make room for the dental clinic, which will be located in the upstairs portion of the Faith Community Health Center building, behind the current front desk and reception area. Construction plans include two dental exam rooms and a full dental lab, as well as a renovated waiting room and reception area.

“We are excited about the progress the dental clinic has made thus far and to be able to start seeing dental patients,” said Dr. Michelle Scholtz, Horizon’s Chief Dental Officer. “This is a wonderful opportunity for the community of Faith and the surrounding area, as we continue to provide close-to home access to primary, and now, oral health care in a cost-effective setting.”

The clinic will be open four ten-hour days, Tuesday through Friday each week. Mark Kampfe, DDS, and Jessica Lawson, Registered Dental Hygienist, have been hired to lead the dental team and provide the oral health services. “Dr. Kampfe and Jessica both have extensive previous experience in the Rapid City area and will be an asset to our Faith Dental team,” said Dr. Scholtz.

Appointments will be available starting September 19th by calling the Faith Community Health Center at 605-967-2644. A grand opening celebration will be held on Thursday, November 3rd from 11:30 am – 1:30 p.m., which the community is invited to attend. More information will be released in the coming weeks as the dental clinic prepares to open.

Karley Fischer, PA-C Joins De Smet Community Health Center

(DE SMET, SD) – Horizon Health Care, Inc. (Horizon) is pleased to introduce Karley Fischer, PA-C, as the newest medical provider at De Smet Community Health Center. Karley joins providers Jodi Jung, CNP, Lindy Holland, CNP, Dannica Callies, DNP, NP-C, and Dr. Lance Lim to provide primary care medical services at the De Smet CHC.

A graduate of University of North Dakota in Grand Forks, ND, Fischer holds a Master’s Degree in Physician Assistant Studies and brings eight years of experience in the medical field to her position at De Smet Community Health Center.
Prior to joining Horizon, Fischer has been previously employed in De Smet as an Athletic Trainer and Physical Therapist Assistant. Most recently, she was employed at Avera Medical Group in Brookings as a Physician Assistant in Acute Care. “After graduating and working as an athletic trainer, along with spending time on the local ambulance crew, I was always the initial person that arrived to the injured individual first and I found myself wanting to know the end results,” said Fischer. “I wanted to learn more about the medical field and being able to treat and care for the patients. Therefore, I made the decision to return to school as a Physician Assistant.”

A Huron native, Fischer currently resides in De Smet with her husband, Brian, and daughter, Anikyn. In her spare time, she enjoys being on the farm riding horse, running, spending time at the lake fishing, and being with family.
Fischer will be available for appointments beginning the week of September 26th. “I am looking forward to working at De Smet Community Health Center in the small, rural community where my family lives and farms,” said Fischer. “I have had the opportunity to meet many great people in the community while living here and serving as an EMT in the community, so I am excited to now be able to provide great patient care through Horizon.”

Horizon Health Care, Inc. Receives Nearly $60,000 for Quality Improvement Efforts

(HOWARD, SD) – In a recent announcement by the U.S. Department of Health and Human Services, Horizon Health Care, Inc. (Horizon) has been named as one of five health centers in South Dakota to receive funding dedicated to recognized their health center’s quality improvement efforts. Nationwide, more than $100 million was awarded to 1,304 health centers across the United States.

“Horizon is very pleased to be recognized again for the high quality of care our patients receive through this quality improvement award,” said Lindsey Karlson, Horizon’s Director of Quality Improvement. “As community health centers, our clinicians, nurses, and support staff provide care to patients from diverse backgrounds and rural communities, many of whom would not have access to healthcare otherwise. This award acknowledges that rural healthcare quality can compete and many times exceed the quality of healthcare provided in more urban areas.”

Locally, Horizon will receive nearly $60,000 in recognition of their continuous quality improvement efforts. Health centers receive these funds based upon high levels of performance in one or more of the following categories: Improving Quality Care, Advancing Health Equity, Maintaining Patient-Centered Medical Home (PCMH) Recognition, Enhancing Access to Care, and Delivering Value. This funding comes from the Affordable Care Act’s Community Health Center (CHC) Fund, which was extended with bipartisan support in the Medicare Access and CHIP Reauthorization Act (MACRA) of 2015.

These quality improvement awards are designed to support health centers in continuing to deliver health care that engages patients, improves care coordination, and enhances overall access to care.

Horizon Receives Grant to Improve Rates of Breast and Cervical Cancer Screening in Target Communities

Horizon Health Care, Inc. (Horizon) has been awarded a grant from the South Dakota Department of Health for $15,000 to increase breast and cervical cancer screening rates in the communities of Aberdeen, Fort Thompson and Wessington Springs, SD.

Horizon will be working with the South Dakota Department of Health, and its Comprehensive Cancer Control Program and Breast and Cervical Cancer Early Detection Program to develop strategies for ensuring all women of appropriate age are screened.

According to the South Dakota Cancer Registry, cancer was the second leading cause of death in South Dakota in 2012 with more than 4,180 cancer cases diagnosed and 1,623 deaths. In 2012, the most common cancer diagnosis in the state was female breast cancer. Each day, 11 cases of cancer are diagnosed in South Dakota residents.

“Breast and cervical cancer can affect women of all ages, even without symptoms,” said Kathy Hettinger, CFNP, Aberdeen Community Health Center. “As a lifelong advocate for women and their health, I encourage all women to visit with your healthcare provider about what routine screenings you need. Screening is simple and easy. Ask questions and get screened, not only for you, but for your family and loved ones.”

Horizon will utilize grant funds to increase breast and cervical cancer screening rates by increasing awareness for the need for screening, sending reminders to patients who need to complete screening and working in partnership with clinicians to review rates of screening and develop strategies to increase screening rates among targeted populations.

Horizon Health Care, Inc. to Expand Oral Health Services in Faith, SD

US Department of Health and Human Services awards $1,049,999 to health centers in South Dakota to expand oral health services

(HOWARD, SD) – In a recent announcement by the U.S. Department of Health and Human Services, Horizon Health Care, Inc. (Horizon) has been named as one of two health centers in South Dakota to receive funding to expand oral health care services. The funding will increase access to integrated oral health care services and improve oral health outcomes for Health Center Program patients. Nationwide, nearly $156 million was awarded in funding to support 420 health centers in 47 states, the District of Columbia and Puerto Rico.

Locally, Horizon will receive $525,000 in funding and plans to use this to establish a new dental clinic in Faith, SD. In collaboration with community partners, Horizon will expand oral health services in their existing primary care clinic located in Faith, to improve the availability of and accessibility to quality oral health care services for the patients. This expansion will bring oral health services to a community and surrounding service area that has not had dental services locally for well over a decade. Currently, the closest oral health care facility accepting uninsured patients is over 125 miles from Faith.

“We are pleased to expand our oral health care services into the Northwest region of South Dakota with this funding,” said John Mengenhausen, Horizon CEO. “In South Dakota, dental disease and lack of access to dental care are critical health care issues. We knew there was especially a need in Faith and Meade County, as well as the surrounding areas, so we look forward to the opportunities this brings for the area.”

The new dental clinic will be located within the current Faith Community Health Center and will be opening in late October 2016.
