An account is established for each family. We request that you pay for your health care at the time of service. Eliminating statements helps keep our fees lower.
  • Private Insurance: We will file insurance claims for all services unless you request otherwise. Insurance rarely pays the full amount, so we ask that you pay the co-pay and deductible at the time of service. If payment from the insurance company results in an over-payment on your account, a refund check will be issued to you.

  • Self-Pay: Full payment is expected at the time of your visit. A discounted fee schedule or "sliding fee scale" is available to patients who meet the income guidelines. Please ask the receptionist for more information and an application form. Any discounts for which you may qualify will be applied to your bill.



    • Horizon Health Care accepts Mastercard and Visa as payment options at each clinic.

       

  • Medicare: Horizon Health Care clinics bill Medicare for services rendered to you. After Medicare payment is received, we will submit the balance to your supplemental insurance company. If you don't have a Medicare supplement, you will be billed for the balance due. You may wish to apply for the sliding fee scale or ShareCare, a program of the South Dakota State Medical Association which provides free medical assistance for people 65 and over who are on limited incomes.

  • Medicaid : Patients on South Dakota Medicaid are required to present their identification card at every visit. Horizon Health Care clinics will bill Medicaid for any services rendered.

  • CHIP: HHC clinics will bill the SD Department of Health for services rendered to children enrolled in the Child Health Insurance Program (CHIP). Patients are required to present their identification card at every visit.
