For over 35 years, Horizon Health Care, Inc. has been providing personalized, affordable, high-quality medical, dental and mental health care through a rural, community-based network of 17 community health centers throughout South Dakota.
Horizon serves South Dakotans in rural, medically underserved areas through 72,000 patient visits annually in the following communities: Bryant, DeSmet, Howard, Huron, Lake Preston, Martin, Mission, Plankinton, Springfield, Wessington Springs, White River and Woonsocket.
Through our Sliding Fee Program, partnerships with providers in neighboring areas and the latest in Telemedicine technology, we continue to provide access to comprehensive medical and dental services, as well as financial assistance resources to all members of the communities we serve.
