Horizon Health Care has provided 30 years of dedicated, culturally-sensitive care to nearly 16,000 low-income, underinsured and uninsured South Dakotans. Our community-based clinics are portals to high-quality care and a higher quality of life regardless of a patients ability to pay.
Since our inception, Horizon has consistently provided healthcare and human services to rural underserved communities of South Dakota. It is our hope, indeed it is our mission, to serve and respond to the needs of families by providing exceptional health programs.
